Eligibility:
You must be a US citizen
You must be in treatment for cancer (LIGHT helps all cancer patients regardless of which cancer is diagnosed)
You must have insurance that helps pay for treatment.
You must have documentation from your physician confirming diagnosis.
You will need to submit to a credit check to determine need.
Process:
Print our Financial Assistance Application and fill it out in detail.
Make sure you have provided all required information.
Submit copies of bills or estimate of treatment costs from you doctor or other medical facility for treating your cancer.
Submit a brief description of yourself and your needs.
Once everything is complete it will be sent to a Patient Assistance Committee for review and approval based on need and finances.
Who LIGHT pays:
LIGHT will make payments directly your physician or medical facility.
LIGHT will make arrangements with your pharmacy for the payment of prescriptions if approved.
LIGHT does not make any payment directly to a patient.
Funds are for cancer treatments only.
